Successful Management of Severe Paraquat Poisoning with Free Radical Scavenger



Abstract
The purpose of this paper is to report three cases of severe paraquat intoxication treated successfully with multiple antioxidants combination therapy of superoxide dismutase inhalation by nebulizer, intravenous administration of aminoacids,
vitamin C
and E.
Three cases of the patients with paraquat intoxication were treated with Superoxide dismutase inhalation by nebulizer, packed RBC transfusion, intravenous injection of aminoacids, Vitamin C and Vitamin E.
Case-1 was a 35 year old woman, swallowed 6 mouthfuls of 20% paraquat for the purpose of suicide. The plasma paraquat concentrations were 2133ng/ml at 2 hours retrospectively.
Case-2 was a 50 year old woman, swallowed two mouthful of 20% paraquat.
The plasma paraquat concentrations were 3522ng/ml at 6 hours, 1358ng/ml at 12 hours, 85 ng at 18 hours, and 8 ng/ml at 24 hours retrospectively.
Case-3 was a 55 year old man who drank 2 mouthfuls of 20% paraquat.
The plasma paraquat concentrations were 1172ng/ml 10 hours, 189 ng/ml at 16 hours, and 16ng/ml at 22 hours retrospectively.
All of the three cases were treated successfully and discharged without specific complication except exertional dyspnea in case.
We found that the mortality of paraquat intoxication could be decreased by various efforts to improve free radical scavenger which are easily available in clinic. Further controlled study is necessary to prove it.
